Fixes to docs now that LTI1.3 is in production.
Please check & edit the instructions re: contacting Runestone with questions re: LTI integration and domain approvals. Currently two spots point to making an issue on github. If that is what you want, "LTI 1.3 Integration" needs to be made into an issue type. If not, feel free to edit.
