- Add EnablePostGISMigration to automatically create/drop PostGIS extension
- Include EnablePostGISMigration as first migration in test suite
- Add SQLKit import for raw SQL execution
- Define FluentPostGISSrid constant (4326) for spatial reference
- Fix trailing comma in migrations array
